Dispute Resolution
The hockey environment can be quite emotional in both a positive and negative sense. As a spectator at a hockey game or practice, it is common for individuals to pass judgement and opinion on the situation at hand and/or the hockey program. At times, these judgements and opinions may be accurate or inaccurate. When these views evolve into a dispute, it is highly advisable to address the matter at the earliest opportunity. Below is the form to submit a formal complaint.

Name of individual(s) against whom you are complaining. (if not known to you then please provide a description of the individual and the role he or she played during the incident.

Please provide the following details with respect to your complaint.

Please Provide a descriptive summary of the incident(s) leading to this complaint.

Section F

Please Provide the Names and contact information of anyone who was a witness to the incident(s) you have described above

I confirm that the information completed in this form are accurate and truthful and are free from any mal-intent

I understand that this form will be sent to our Director of Risk Management and information will be shared with the Dispute Resolution Committee. A follow up will be issued within 3 business days or earlier depending on the nature of the complaint.

Follow up emails will not be responded to during the time frame mentioned above
