House League Coach and Trainer Applications
Without dedicated coaches and trainers there is no hockey.
Please consider returning as a coaching/training volunteer or joining as a new coach/trainer. All teams must have a head coach and a trainer - they cannot be the same person.

Completion of this application form does not guarantee you acceptance for a position within the BMHA.
All accepted House League Officials (i.e., Coach, Assistant Coach, and Trainer) MUST provide a current police clearance (e.g., CPIC) for the current season by November 1st. Failure to do so will result in removal from the bench.
Certification Requirements can be found on the Coaches Corner section of the website.
The information contained on this form shall be used for the sole purpose of making application to the BMHA for a House League Coaching/Training position, confirming the necessary police clearance, and for no other purpose whatsoever.
